Genetic Data Processing Consent
Please read this notice before registering your kit. Checking the box on the registration form is how you agree.
1. Who you are authorizing
Test For Life is the organization providing this kit. The Kure (KureAI Inc.) is a subprocessor that Test For Life uses to process genetic data.
2. What you are consenting to
You request genetic testing. You authorize Test For Life and its subprocessors to: receive your sample (for example, saliva); extract DNA; sequence and analyze genetic information; generate a report; store data as needed to deliver and support the service; and share results with Test For Life and, where applicable, the clinic or ordering provider associated with your kit.
3. Subprocessors
Processing may involve CLIA- or CAP-accredited laboratories, cloud hosting, and other vendors bound by written data-protection terms. Because this form is provided by Test For Life, The Kure is one of those subprocessors.
4. Purpose and limits
This consent is only for providing the testing service you are registering for. It is not consent for unrelated research or for the sale of identifiable genetic data.
5. Voluntary
Genetic testing is voluntary. If you do not check the box on the registration form, we cannot register this kit or process your sample.
6. Results and limitations
Reports are not a diagnosis or a substitute for medical advice. Discuss results with a qualified clinician. Testing has limitations, including variants that may not be reported and the possibility of a failed sample.
7. Privacy
Genetic information is sensitive. Test For Life and its subprocessors will handle it under applicable law (including HIPAA where it applies) and the privacy practices of Test For Life.
8. Retention and withdrawal
Data may be retained as required for the service, quality, and law. You may request to withdraw consent for future processing by contacting support. Withdrawal does not undo processing already performed and may mean we cannot complete or update your report.
9. Your confirmation
By checking the box on the registration form you confirm you are the person identified (or are legally authorized to consent for them), you have had a chance to read this notice, and you agree to the processing described here.
